Missed a GLP-1 Dose? What Each Label Says to Do

Missed your Ozempic, Wegovy, Mounjaro or Zepbound dose? What each FDA label says about late doses, skipped weeks and restarting, and who to ask next.

Written by Holt Editorial Team4 min read

It's Wednesday evening, you open the fridge for something else, and there's the pen, still in its box. Your shot day was Sunday. Now you're doing mental arithmetic: is it too late, should you take it now, will tomorrow's plans fall apart if you do? The good news is that every weekly GLP-1 label answers this question directly. The catch is that they don't all give the same answer.

Is a missed dose a big deal?

One missed week is something the labels plan for, which is why each one has a short missed-dose section. What matters is following the rule for your own medication rather than one you read about for a different one. The windows differ by brand, and so does the way they're counted.

Everything below is quoted or closely summarized from the current FDA prescribing information and Medication Guides on DailyMed. It isn't personal advice. If your situation doesn't fit neatly, your pharmacist can answer it in a two-minute call.

What does each label actually say?

MedicationIf you miss a doseIf it's too lateAlso in the label
Ozempic (semaglutide)Take it as soon as possible within 5 days after the missed doseSkip it; take the next dose on your regular dayYou can change your shot day if your last dose was 2 or more days before
Wegovy injection (semaglutide)Take it as soon as possible if your next scheduled dose is more than 2 days (48 hours) awayIf the next dose is less than 2 days away, don't take it; take the next dose on your regular dayMissed 2+ weeks in a row: contact your provider about restarting
Wegovy tablets (semaglutide)Skip the missed tabletTake the next dose the following dayDaily pill, so there's no catch-up window
Mounjaro (tirzepatide)Take it as soon as possible within 4 days (96 hours)Skip it; take the next dose on your regular dayDon't take 2 doses within 3 days (72 hours) of each other
Zepbound (tirzepatide)Take it as soon as possible within 4 days (96 hours)Skip it; take the next dose on your regular dayDon't take 2 doses within 3 days (72 hours) of each other

Notice the Wegovy injection rule counts differently. Ozempic, Mounjaro and Zepbound count forward from the dose you missed. Wegovy counts backward from your next scheduled dose. They often land in a similar place, but if you're near the edge, read your own Medication Guide or ask the pharmacist rather than borrowing another brand's number.

What if I've missed more than one week?

This is where the labels say the least, and where you most need your prescriber.

The Wegovy label is the only one of these that spells it out. The Medication Guide says that if you miss doses for 2 or more consecutive weeks, you should contact your healthcare provider to talk about how to restart. The prescribing information tells prescribers to restart the dose escalation at a lower dose, to reduce the risk of stomach side effects.

The Ozempic, Mounjaro and Zepbound labels we checked don't give separate guidance for several missed weeks. That isn't permission to jump straight back to your old dose. It means the label doesn't say, and the decision belongs with your prescriber. The Ozempic, Mounjaro and Zepbound labels report that most nausea, vomiting and diarrhea in their trials happened during dose escalation, which is part of why a restart plan matters. Our dose escalation guide explains how the steps usually work.

Can I move my shot day instead?

Sometimes, yes, and the labels say how. The Ozempic and Wegovy guides say you can change the day of the week as long as your last dose was 2 or more days before. The Mounjaro and Zepbound guides allow a change as long as there are at least 3 days (72 hours) between the two doses.

Moving your day on purpose is different from drifting. If a late dose becomes your new shot day, tell your prescriber at your next check-in so their records match yours.

Why do doses get missed in the first place?

Usually for ordinary reasons: travel, a busy week, a pharmacy delay, or feeling unwell and putting it off for a day that turns into four. Sometimes a procedure is coming up; the labels ask you to tell your healthcare provider about any planned surgery, and any change to your doses around it is their call.

Whatever the reason, write it down. A pattern of misses around the same weekday, or always after a dose step, is useful information for you and your prescriber.

What we'd track

  1. Your shot day and time. A clear "next dose" countdown makes it obvious when you're inside a window and when you're not.
  2. Every dose as it happens. If you're unsure whether you took last week's shot, a log answers it in seconds, before you risk a double dose.
  3. Late or skipped doses, with a reason. "Took it Thursday, traveling" is enough. It helps your prescriber judge anything that follows.
  4. Side effects after a late dose or restart. If nausea flares after a gap, that's worth knowing and worth reporting.

Holt reminds you on your schedule and puts a Live Activity on your lock screen when it's shot time, so the pen is harder to forget. The treatment screen shows your last dose and a countdown to the next one, and your dose history keeps every dose with its strength, injection site and an adherence count, like 12 of 12. Holt won't tell you whether to take a late dose — that's your label and your pharmacist — but it makes the "when did I last take it?" question easy to answer. More on that in our GLP-1 dose tracker guide.

Frequently asked questions

How late can I take a missed Ozempic dose?

The Ozempic label says to take the missed dose as soon as possible within 5 days after the missed dose. If more than 5 days have passed, it says to skip it and take the next dose on your regularly scheduled day.

What does the Zepbound or Mounjaro label say about a missed dose?

Both say to take the missed dose as soon as possible within 4 days (96 hours). If more than 4 days have passed, skip it and take the next dose on your regular day. Both Medication Guides also say not to take 2 doses within 3 days (72 hours) of each other.

I missed two or more weeks of Wegovy. Can I just pick up where I left off?

Don't decide that alone. The Wegovy Medication Guide says that if you miss doses for 2 or more consecutive weeks, contact your healthcare provider to talk about how to restart your treatment. The prescribing information tells prescribers to restart the dose escalation at a lower dose to reduce the risk of stomach side effects.

What if I miss a Wegovy pill?

The Wegovy label says to skip the missed tablet and take the next dose the following day, then carry on with your regular schedule.

Should I take two doses to catch up?

None of these labels tell you to double up. The Mounjaro and Zepbound guides specifically say not to take 2 doses within 3 days of each other. If you're unsure, call your pharmacist.
