Semaglutide Tracker — Weekly Shots, Daily Pills & Compounded Vials

How to track semaglutide whether it comes as a weekly pen, a daily tablet or a compounded vial — what each one's rules ask you to log, and why units aren't mg.

Written by Holt Editorial Team

Worth remembering

  • Start your log by writing down exactly which product you're on — brand, form and strength — because the rules differ.
  • Weekly injections (Ozempic, Wegovy) and daily tablets (Rybelsus, Wegovy pill) have different missed-dose rules.
  • The two Rybelsus formulations aren't interchangeable mg for mg, so the label's own tables matter if you switch.
  • With compounded vials, record both the mg dose and the syringe units your pharmacy specified; FDA has reports of dosing errors tied to mixing these up.
  • FDA says compounded drugs aren't FDA-approved and aren't reviewed for safety, effectiveness or quality before they're sold.

Your prescription says "semaglutide." The pharmacy label says something else — maybe Ozempic, maybe Wegovy, maybe Rybelsus, maybe a vial with a pharmacy's name on it and a dose written in units. Same molecule, four very different routines. Before you can track semaglutide well, you need to know which one you're holding.

Which semaglutide are you on?

Semaglutide comes in several forms in the US, and each has its own label (or, for compounded versions, no FDA label at all):

ProductFormHow oftenWhat it's approved for
OzempicInjection penOnce weeklyType 2 diabetes (with heart and kidney indications)
Wegovy injectionPen or syringeOnce weeklyWeight reduction, cardiovascular risk reduction, MASH
Wegovy tabletsTabletOnce dailyWeight reduction, cardiovascular risk reduction
RybelsusTabletOnce dailyType 2 diabetes
Compounded semaglutideUsually a vialAs prescribedNot FDA-approved

The first line of any semaglutide log should be the product name, form and strength exactly as they appear on the box or bottle. Everything else follows from that.

What changes between a weekly shot and a daily pill?

Almost everything you'd log.

Weekly injections. Both Ozempic and Wegovy are injected once a week, on the same day, any time, with or without food, rotating between stomach, thigh and upper arm. Their missed-dose rules differ, though. The Ozempic label says to take a missed dose within 5 days. Wegovy's rule turns on whether your next dose is more or less than 2 days away. If you ever switch between them, the rule you remember may no longer be the one that applies.

Daily tablets. Rybelsus and the Wegovy pill share a morning ritual: an empty stomach, up to 4 ounces of plain water, the tablet swallowed whole, then at least 30 minutes before food, drink or other oral medicines. For a missed tablet, both labels say to skip it and take the next one the following day.

What's different about compounded semaglutide?

Compounded semaglutide is made by a pharmacy rather than the brand manufacturer. FDA says plainly that compounded drugs are not FDA-approved, and that the agency doesn't review them for safety, effectiveness or quality before they're sold.

FDA treated semaglutide injection as being in shortage for a period, which allowed wider compounding. In February 2025 it announced that the shortage was resolved, and it set wind-down dates for compounders after that. FDA's guidance now says compounded drugs should be used only when a patient's medical needs can't be met by an FDA-approved drug.

For tracking, the important part is how the dose is measured. FDA has received adverse event reports, some leading to hospitalization, that may be linked to dosing errors with compounded injectable semaglutide. These include people measuring the wrong amount and some health professionals miscalculating doses. Vials are drawn up with a syringe, and syringes are marked in units or mL, not mg. How many units equal your dose depends on that vial's concentration.

FDA's consumer advice also covers handling. Get your prescription filled at a state-licensed pharmacy, don't use an injectable GLP-1 that arrives warm, use a new sterile needle and syringe every time, and throw away multi-dose vials within 28 days of first use.

What we'd track

Whichever semaglutide you're on, these are the entries we'd want in the log:

  1. Product, form and strength. "Wegovy pen 1 mg" or "compounded, 2.5 mg/mL vial." This is what makes the rest of your history readable.
  2. Dose date and time. Weekly or daily, this is the basis for every missed-dose rule.
  3. Dose in mg — and, for vials, the units drawn. Keep both side by side, exactly as your prescriber or pharmacy wrote them. If they don't match what you expected, call the pharmacy before injecting.
  4. Injection site or morning timing. For shots, note where you injected. For tablets, note when you took it and when you first ate.
  5. How you felt. A short note on nausea, appetite or energy in the days after each dose or dose step.

Holt is built for this mix. You can log weekly shots or daily pills, set reminders with a Live Activity, record injection sites, and line up side effects against dose days. Holt records the dose you log; the units to draw come from your pharmacy label and your pharmacist's instructions.

Frequently asked questions

Is semaglutide the same as Ozempic?

Semaglutide is the active ingredient; Ozempic is one brand of it, approved for type 2 diabetes. Wegovy (injection and tablets) and Rybelsus are other FDA-approved semaglutide brands with different labels.

How many units of compounded semaglutide is my dose?

That depends on the concentration of your specific vial, so only your prescriber or pharmacy can tell you. Write the mg dose and the number of units they gave you on the same line, and ask if the vial or concentration ever changes.

How long can I use a compounded semaglutide vial?

FDA's consumer guidance says to discard multi-dose vials within 28 days of first use, and to use a new, sterile needle and syringe for each dose. Follow your pharmacy's instructions if they're stricter.

Can I take more than one semaglutide product at a time?

The Wegovy label says using it with other semaglutide-containing products or any other GLP-1 medicine is not recommended. If you've been prescribed more than one, check with your prescriber.
